For your convenience, Dominican Medical Foundation has six locations throughout Santa Cruz County to serve you. In addition, we have specialists in Geriatrics, Endocrinology, Palliative Care, Surgery and Hospital Medicine. Please discuss your options with your physician.

Our office hours are generally 8 a.m. to 5 p.m., Monday through Friday. For your convenience, some of our physicians offer extended hours. Dominican Medical Foundation also offers patients the added convenience of same-day appointments with most of our physicians.
Our team of specialty trained hospital physicians work in the hospital to help you through your hospital stay. Working closely with appropriate specialists, they assure that you receive high quality, properly coordinated care.
Dominican Medical Foundation is affiliated with Catholic Healthcare West and Dominican Hospital.
To refill a prescription, please contact your pharmacy directly and they will take care of contacting your physician to fill your prescription. Please allow 48 hours for all refill requests.
