How do I select a Dominican Medical Foundation physician? If your insurance requires the selection of a primary care physician (PCP), call your health plan and notify them of your new PCP selection.

How do I make an appointment? To schedule an appointment, contact our office(s) directly. Phone numbers can be found by clicking here.

What should I bring to my first visit with my doctor? You should bring a copy of your insurance card, a list of current medications, your applicable co-payment, and information about your medical history.

What if I need care after normal business hours? If you are having a life-threatening emergency, you should immediately call 911. For all other health-related calls, Dominican Medical Foundation has a physician on-call, 24 hours a day, seven days a week, 365 days a year. You may call our regular phone numbers, and you will be directed to a physician. For all other calls (i.e., billing, appointments, refills, etc), please call during normal business hours.

May I request copies of my medical records? Yes. Copies of medical records can be requested during normal business hours.
